Your GP or health professional can refer you to our health and wellbeing referral programme, where we’ll help you become more active through enjoyable and engaging activities.

If you are managing a long term condition such as diabetes, Parkinson’s, cancer or struggling with your mental health, we offer a variety of activities and classes for everyone, all led by our highly skilled and qualified team. Throughout the programme we’ll support you in your journey to leading a healthier lifestyle. We’ll provide you with expert support which will be centred on your interests and reason for referral.

It doesn’t matter if you’ve never been an active person. We have options for all abilities. 

By increasing or maintaining good levels of activity, you can:

  • Delay or prevent chronic illnesses and diseases associated with inactivity such as heart disease, stroke, type 2 diabetes, and some types of cancer.
  • Maintain your quality of life and independence longer as you age.
  • Boost your energy levels so that you can do more.
  • Manage your mental wellbeing.
  • Improve your strength and balance so you can prevent injuries and stay independent.
  • Lower your pain.
  • Create healthy social connections.

Frequently Asked Questions

You can access 12 weeks of support into the physical activity referral programme if you are living with or at risk of a long term physical or mental health condition. 

You’ll be able to attend gym support groups, move more circuits and gentle movement sessions. 

Our gym support group offers weekly sessions where a member of our health and wellbeing team will be on-hand to offer support during your gym sessions. They’ll make adjustments to your plan and help you build confidence so that you feel able to independently increase your level of activity. 

1. You can get referred into the programme by Your GP or another healthcare professional.

You should speak to your GP about a physical activity referral, they’ll be able to start the referral process for you. 

2. If you have had cancer or received treatment for cancer, you can self refer via this form.

3. If you are living with Parkinson’s, speak to your GP, physio or Parkinson’s nurse about a referral. 

Once we’ve receive your referral, a member of our health and wellbeing team will be in touch to: 

  • Ask about what you hope to get from the programme 
  • Chat about what the different option can look like – weekly group or independent access for 12 weeks

This process can take up to 12 weeks once we receive your referral.
